Coming from the West on interstate I-80:

1) Take the H Street exit
2) Turn right at H Street
3) Turn right at the 2nd cross street onto Alhambra Blvd
4) Turn left at J Street
5) Turn left on 33rd Street
6) Turn right into the first driveway on the right

Coming from the East on interstate I-80:

1) Take the J Street exit
2) Turn left at J Street
3) Turn left on 33rd Street
4) Turn right into the first driveway on the right

By bus: The number 30 stops near the clinic on J Street.

For New Patients

Please arrive 30 minutes early if you are a new patient and this is your first appointment. If you are not able to keep your appointment, phone us as soon as possible to cancel. Please bring the following to your appointment:

All of your medication bottles
Medical records
Immunization records
If medical records are unavailable, please bring the names, phone numbers, and addresses of former medical providers.

Returning Patients

If you are not able to keep your appointment, phone us as soon as possible to cancel. Please bring the following to your appointment:

All of your medication bottles
Glucometer if diabetic
Peak flow meter if you use one

Saturday & Sunday - Closed

Monday through Friday from 9:00 AM - 5:00 PM
